Cancer Treatment Center Director (Hosp. Op.) directs and oversees all activities of the hospital's cancer center. Targeted Case Manager

  • TrueNorth Treatment Center is looking for a targeted case manager to enhance the quality of client management. The appropriate candidate will be accountable for ensuring resource planning, advocacy efforts, and continued monitoring of each assigned client's care. Case manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human services needed. Targeted Case Management Certification is a plus.

    Responsibilities

    • Develop a strengths-based needs assessment and periodic reassessment for each client served in accordance with state and federal criteria.
    • Develop an individualized care plan for each patient served in accordance with state and federal criteria.
    • Work with the patient and/or patient’s family to address issues related to implementation of their care plan.
    • Assess the effectiveness of the care plan in meeting the identified needs of the recipient and make changes as needed.
    • Link the recipient with services and resources identified in the care plan.
    • Advocate for the acquisition of services and resources necessary to implement the care plan.
    • Commitment to high-quality, ethical practice
    • Openness to providing case management and resource coordination services in a variety of settings (e.g. office, schools, homes)
    • Desire to work with individuals and families facing multiple challenges
    • Ability to work effectively with adults diagnosed with a Serious Mental Illness (SMI), Substance Use Disorder (SUD), and Co-Occurring Disorders (COD). This may include working with children with Severe Emotional Disability (SED).
    • Willingness to work collaboratively as part of a multidisciplinary team
    • Effective time management and documentation skills.
    • Provide service linkage and referrals to clients in Continued Care programming.

    Skills

    • Proven working experience in case management, including as a nurse, medical, mental health care manager or a related job
    • Previous experience working with substance-use, several mental illnesses, and/or serious emotional disabilities.
    • Effective written and verbal communication skills
    • Excellent organizational and time management skills
    • Problem solving skills and ability to multi-task
    • Compassionate with teamwork skills
    • Current license, certification or registration
    • Bachelor’s degree in behavioral science, human services or a related field
    • Two years of experience working with individuals in a human service setting
    • Ability to meet applicable requirements as a Medicaid provider
    • Current certifications in TCM for SUD, SMI, SED, and/or COD
    • Must pass background check and drug screen
    • Must possess an active CSW/LSW licensure if applicant has a social work degree.

    Job Types: Full-time, Contract

    Pay: From $55,000.00 per year
