We are seeking a Mental Health Practitioner to work with adults who have a serious and persistent mental illness (Case Management) or serious mental illness (ARMHS). Case Management services help adults with a serious and persistent mental illness gain access to needed medical, social, educational, vocational, financial and other necessary services as they relate to the recipient’s mental health needs. Adult Rehabilitative Mental Health Services (ARMHS) promotes independent living skills.
Duties and Responsibilities:
· Complete all paperwork, reports, and charting contemporaneously and in an organized manner. Maintains good writing skills.
· Must maintain good communication skills.
· Schedule face to face visits as dictated by the Individual Community Support Plan or Treatment Plan.
· Make appropriate / culturally sensitive referrals and assist clients in gaining access to needed services.
· Coordinate with team members their responsibility for arranging the services necessary to maintain and enhance each client’s ability to live as independently as possible.
· Attend scheduled reviews, team meetings and clinical supervision regularly.
· Provide education to clients regarding the symptoms, course and treatment of their illness.
· Comply with the DHS Rule 79 and/or ARMHS guidelines.
· Comply with the necessary continuing education requirements.
· Other duties as assigned.
Requirements:
Must have a minimum of a Bachelor Degree for Case Management in social work, psychology, human service or behavioral science related field and one year / 2000 hours of experience working with adults with mental illness.
Must be able to pass a background study.
Must have a valid driver’s license and auto insurance.

Pay: From $25.00 per hour
