Job Summary
Total Health Care has a commitment to provide a full range of services that meet the needs of the patients; this includes a continuum of services that include behavioral health specialty (mental and substance use disorders) and infectious disease services. There is also a complement of supportive services within the Division of Integrated Behavioral Health (IBH), to include community health workers, case managers and outreach services for patients receiving services within IBH.
This position provides program oversight for case management services to include all Ryan White Grant funded services; as well as case management for all of behavioral health services. The Supervisor is responsible for maintaining the principles and standards for the delivery of the service categories listed. This will include hiring, training and supervising the staff, ensuring compliance with the funder’s standards of care within each of the service categories, as well as, financial management of the service categories. It is also necessary for the supervisor to establish and maintain relationships with community partners and funders.
ESSENTIAL JOB FUNCTIONS

Responsible for the overall quality and supervision of the Case Management department

Provide program development planning and evaluation; including coordination with the oversight of their federal, state, and local grants.

Problem/issue identification, analysis, research and resolution;

Policy and procedure development and revision;

Budget development and monitoring;

Develop annual goals for the program and evaluate effectiveness

Ensure program goals are aligned with grant objectives

Gather and disseminate information from internal program personnel that will ensure understanding of the concept/goals/mission of the department/service for which funding is maintained.

Monitoring/authorizing the quality of staff input and training on the e-requestor system for all electronic requisitions and check requests.

Provide supervision, training and support for program employees
-
Provide training and leadership in the service delivery model and implementation across all departments.

Develop and revise staff job descriptions when changes in the program occur.

Ensure staff time is accurately assigned to proper departments and time and effort is maintained.

Responsible for annual staff evaluations/staff development planning and quality of the staff development implementation.

Prepare timely monthly departmental reports

Participate in team management of agency and development of long term planning goals and objectives

Facilitate department meetings

Participate in IBH Management Team meetings

Oversee special projects in the department

Support department lead positions in their roles

Ensure consistent communication with offsite employees

Address client concerns and complaints appropriately

Maintain and nurture relationships with key funders, policymakers, supporters and community influencers related to client services

Coordinate with the Program Manager in supplying data and all reporting requirements when contractually required by all funders.

Assist the Program Director in measuring current baseline performance/gaps in performance in service categories for developing a Grants Administration Strategic Plan.

Work with IBH Directors to provide input for all written organizational materials (including annual report, newsletter, and THC Corporate website).

Ensure compliance with HIPAA and all related confidentiality/privacy legal requirements

Stay up-to-date with RWCA legislation and regulations pertaining to HIV case management

Stay up-to-date with HIV/AIDS disease and treatment issues

Maintain a caseload of a minimum of 15 patients towards clinical license qualifier.
MINIMUM EDUCATION AND EXPERIENCE REQUIRED
Masters Degree in Social Work with an LGSW and three years progressive program management experience in Human Services Management and/or medical setting.

WORK ENVIRONMENT
This position involves everyday risks and discomforts, which require normal safety precautions typical of such places as offices, meetings, training rooms, and other THC sites. The work area is adequately lit, heated, and ventilated.
