Case Manager - Alcohol or Drug jobs in Goose Creek, SC

We are seeking a skilled Substance Use Disorder Counselor to help provide clinical services in an Outpatient Treatment setting . In this role, you will be responsible for providing client- centered intervention, recovery, treatment, and prevention services. The role consist of but not limited to:

• Managing patient care from date entry to discharge.
• Conducting assessments and developing appropriate treatment and recovery plans.
• Facilitating individual and group therapy sessions in an outpatient setting.
• Monitoring patients over time to access treatment progress and need for higher levels of care.
• Collaborating with referral sources, physicians, families and patient support-network.
• Attend trainings and using evidence-based practices.
• Conducting drug and alcohol testing to monitor treatment plans and medical instructions.
• Entering data daily into the EHR.

Required: Bachelors Degree in a human service field and 1 year experience is required, and/or a Master Degree in a human services field and/or ADC, LAC, AADC, BSW, MSW, LPC, LAC, LMSW or LISW.

• Good time management skills.
• At least 1 year experience in counseling.
• Knowledge of EHR
