Additionally, Case Manager - Alcohol or Drug requires a valid Licensed Chemical Dependency Counselor(LCDC) license. Typically reports to a manager or head of a unit/department. The Case Manager - Alcohol or Drug occasionally directed in several aspects of the work. Gaining exposure to some of the complex tasks within the job function.     Greater Nashua Mental Health is looking for a full-time Case Manger to join our Drug Court Team! The Drug Court Case Manager provides high quality case management services to assigned participants, using evidence-based approaches.  Responsible for monitoring and documenting assigned participants' compliance, engagement, and progress with the Drug Court programs, and provides regular updates to the Drug Court Team.

     
     

    Duties & Responsibilities: 

    • Completes case management assessment with assigned participants upon intake and on a regularly scheduled basis, utilizing assessments to develop plans to address barriers to treatment and make progress towards treatment and recovery goals.
    • Works collaboratively with assigned treatment provider, PPO, Recovery Coach, and participant to develop an integrated and individualized case plan targeting the specific criminogenic needs of assigned participants.
    • Provides appropriate, necessary, and effective referrals to ancillary services, while avoiding overlap and redundancy.
    • Monitors, and reports up-to-date information on the status of each assigned participant, including compliance with Drug Court requirements and progress toward Case Plan goals at weekly Drug Court Staffing Meetings or via email and phone.
    • Maintains documentation of each participant's information and case management services in keeping with GNMH, the HCSADC, and the State Office of Drug Offender Programs requirements and standards.
    • Collects and administers drug testing, as required, and consults with the laboratory to interpret results.
    • Communicates professionally, and effectively with colleagues, collateral contacts, supervisors, etc. regarding client services
    • Performs other duties or special projects as required or as assigned. 

    Requirements

    Qualifications: 

    • Bachelor’s Degree in Social Work, Human Services, Criminal Justice or related field.
    • 3-5 years of experience.
    • Strong collaborating skills and an ability to work effectively both independently and as part of a team.
    • Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive.
