Case Manager - Home Care/Hospice coordinates the overall interdisciplinary care plans for home health/hospice patients from admission to discharge.     Supervisor is defined as a highly qualified Case Manager with more than two years of experience. The primary role of a Case Manager Supervisor is to provide direct supervision for Case Managers. Supervisor’s will be responsible for; assessment, planning, coordination, implementation, and evaluation of the Case Managers they supervise. The ideal candidate will be a competent individual who will be able to guide and train employees. They will be well-versed in processes under the role’s responsibility and will be results-driven and focused. Supervisor’s will carry a personal case load based on competency and ability level.

    Responsibilities

    · Personal caseload size of 21 (may be temporarily increased based on need)

    · Will supervise/support 5-8 Case Managers

    · Set goals for performance and deadlines in ways that comply with company’s plans and vision

    · Organize workflow and ensure that Case Managers understand their duties or delegated tasks

    · Supporting Case Managers with questions and concerns: providing ongoing training through, observation of meetings, one on one meetings, and audit reports

    · Monitor Case Manager productivity and provide constructive feedback and coaching through Quality Audits reviewed one on one with each Case Manager supervised

    · Receive complaints and resolve problems

    · Pass on information from upper management to employees and vice versa

    · Prepare and submit evaluations while compiling data and identifying trends and remediation to improve performance

    · Decide on reward and promotion based on performance through Annual Evaluations

    · Assist with hiring and training new Case Managers

    · Ensure adherence to legal and company policies and procedures and undertake disciplinary actions if the need arises

    · Maintain community relations, meeting with providers/stakeholders and participating in transition fairs and events

    · Assist with coverage when needed in the event of a Case Manager on leave

    · Responsible for following the Supervisor Training Guide and the BDDS Quality Guide

    Requirements and Skills

    Strong verbal and written communication skills

    • Ability to conduct annual reviews to discuss job performance, set goals for professional development, establish objectives for contributing to the department’s mission, discuss expectations and accomplishments
    • Attention to detail

    · Proven experience as supervisor or relevant role

    · Familiarity with company policies and legal guidelines of the field

    · Excellent communication and interpersonal skills

    · Outstanding organizational and leadership skills

    Monthly Team Meetings

    · Ability to facilitate the team meeting and team huddle agendas

    Run Audits monthly/weekly (if needed)

    · Open IRs/Open Critical: Review IR follow up. Make sure follow ups are completed every 7 days. Assist CM to get IRs closed within 30 days

    · Review Transition paperwork to ensure that timelines are being met

    · Case Notes – due by the 25th of each month

    · Monitoring Checklists/Incomplete CAPS

    · Missing documents

    · PCISPs/Service Authorizations

    · New Case Assignments

    · Open Onboard/Intakes

    · Rep Payee Actions Needed

    · Running full caseload audits

    · Unannounced visits

    · LOCSI

    · Inactive Medicaid

    · Facility Placement

    · Provider Closures

    Training

    · Shadowing new case managers in individual/team meetings

    · Meet with new case managers once a week to review tasks

    · Attend meetings with Case Managers in person if and when needed for additional support

    · Attend state provided trainings (Life course or downtown trainings provided by the state)

    · Follow 1st/4-week Plan from Orientation

    * Attend vendor fairs as needed

    * Participate and/or organize Community Days.

    Pay: From $60,000.00 per hour
