Case Manager - Home Care/Hospice jobs in New Mexico
Case Manager - Home Care/Hospice coordinates the overall interdisciplinary care plans for home health/hospice patients from admission to discharge. Acts as the liaison between patient/family and homecare personnel to ensure necessary care is provided promptly and effectively. Being a Case Manager - Home Care/Hospice requires a bachelor's degree. Typically reports to a manager. Additionally, Case Manager - Home Care/Hospice requires RN license. Case Manager - Home Care/Hospice's years of experience requirement may be unspecified. Looking to add a Case Manager to our amazing team! Looking for 6 or more years of experience with IDD or a licensed social worker and/or a bachelor's degree in social work, Psychology, Counseling, Nursing, Special Education or a closely related field. You will be working with Individuals with Developmental Disabilities. You will make phone calls, meetings and have monthly visits to address any concerns the Individuals may have. You will be responsible for following up on appointments and any issues that may arise. A Case Manager is required to ensure the individual is receiving the requested services and each of their needs are being met. You will be required to ensure the individuals health and safety. This job requires traveling but transportation and hotel accommodations are provided. At Excel, we truly feel like this is one of the most rewarding jobs you will find.

Pay: $15.00 - $17.00 per hour
