As our RN Case Manager for Acute Care Services (ACS), you will work in the Medically Assisted Therapy (MAT) Outpatient team. You will support complex care for patients in our locations and services that are part of the ACS programs including (but not limited to) Warner House (TRT), Withdrawal Management and various clinics throughout MHP, providing MAT services.
