Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Typically reports to a head of a unit/department. The Case Manager - Mental Health Services occasionally directed in several aspects of the work.     "A Registered Nurse Case Manager for our Acute Care Behavioral Health/Substance Use Disorder Services  is an exciting opportunity to address human needs in the community and use your therapeutic nursing skills to support those struggling with substance use and mental illnesses. You will play an important role, providing nursing services together with an interdisciplinary team of Acute Care Services, the RN Case Manager supports clients with Medically Assisted Therapies (MAT) and behavioral health interventions to engage positively in their communities..     As our RN Case Manager for Acute Care Services (ACS), you will work in the Medically Assisted Therapy (MAT) Outpatient team.  You will support complex care for patients in our locations and services that are part of the ACS programs including (but not limited to) Warner House (TRT), Withdrawal Management and various clinics throughout MHP, providing MAT services.
