Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. This position is full-time with a regular but flexible schedule. An individual who is hired into a Lead D/ID Case Manager position will have a variety of role-specific titles for purposes of funding-source and/or credentialing-body recognition. The role-specific title(s) to be used will be identified in the Lead D/ID Case Manager’s Appointment Letter. Regardless of titles, this position is responsible for the coordination of community-based services that are being provided to individuals that are assigned to the D/ID Case Management Program. The Lead D/ID Case Manager will assist their assigned individuals in gaining access to, and receiving, needed social, medical, educational, vocational, residential, and other services, as directed in the individual’s plan of care. The Lead D/ID Case Manager will assist with training of staff, program compliance and documentation requirements. This position may also be responsible for a variety of tasks that are nonetheless essential under the heading of “Miscellaneous Duties”. Immediate administrative and clinical supervision will vary by program assignment; however, ultimate local supervision will be identified in the individual’s Appointment Letter.
