Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Typically reports to a head of a unit/department. The Case Manager - Mental Health Services occasionally directed in several aspects of the work. What You Will Do to Change Lives
Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.
Shifts Available:
Full Time; 8:30 am - 5:00 pm; Monday - Friday
The expected starting wage range is $17.85 - $21.51. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
What You Bring to the Table (Must Have)- * High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor's degree in Social Services
- Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.

Telecare's Sacramento ARISE program is a Community-Based Program serving approximately 200 members that are adult residents of Sacramento County who have been diagnosed with a serious mental illness. Individuals may be at risk of or experiencing one or more of the following: homelessness, involved in the criminal justice system, have been court-ordered mental health treatment, or are at risk of involuntary psychiatric hospitalization or institutionalization.
