Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. EMBRACE’s Clinical Case Management Program partners with school districts to find residential programs and provide clinical oversight while students reside in placement.
As a Clinical Case Manager (CCM), you will support the school district in determining which residential treatment centers (RTCs) meet a student’s unique educational and mental health needs. You will also manage the referral process and collaborate with the school district as they make appropriate placement offers. The CCM conducts quarterly visits (both in and out-of-state) with the student and RTC team to monitor treatment, milieu, and educational goal progress, while also providing clinical support to caregivers throughout treatment. This clinical oversight, on behalf of the school district, helps to ensure appropriate fit, reasonable length of stay, and a timelier return home for students.
