Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Case Manager (Non-RN) typically requires a bachelor's degree. Typically reports to a head of a department/unit. To be a Case Manager (Non-RN) typically requires 2 to 4 years of related experience. Summary
Participates as a member of a multi-disciplinary team in providing case management/discharge planning interventions to all patients with mental health and substance abuse issues within the BHS system. Provides interventions to mobilize and strengthen internal and external resources to enhance patient's ability to cope with their mental illness and to promote a safe and timely discharge.
Qualifications
Required
- Bachelors degree in related mental health field, preferably Human Services or Case Manager and /or 5 years in-patient mental health experience
- Current unrestricted driver’s license
- Non-Violent Crisis Intervention training within six months of hire
Preferred
- Two years in acute care setting
