Case Manager, Sr. coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager, Sr. evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager, Sr. identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). The Case Manager, Sr. contributes to moderately complex aspects of a project. Work is generally independent and collaborative in nature.     Summary

    We Level Up Treatment Program in Lake Elsinore, California is seeking a Case Manager. The Case Manager is responsible for managing all aspects of the patient’s care, treatment, and services throughout the continuum of care.

    Educational Background and Experience:

    • Mental health or substance abuse experience, at least 2 years
    • Minimum bachelor’s degree in social work, social services, psychology, nursing, or other mental health related field. Individuals with a master’s degree will be given priority. In lieu of a degree, five or more years of relevant experience will be considered.
    • Credentialed or Professional License relevant to degree or field of study will be given priority; credentialing will be encouraged.
    • CPR - Must have prior to the time of hire
    • Must be able to pass a level II background check (AHCA)

    Job Requirements

    • Case manager will ensure that the patient is safely transitioned from admission, treatment, discharge planning and aftercare in a manner that ensures access to the best and most effective treatment and care alternatives.
    • Services include, but are not limited to, biopsychosocial assessment, interdisciplinary treatment planning, participating in interdisciplinary treatment team, coordinating family outreach and family Zoom sessions, social services support, discharge planning, and aftercare follow up.
    • Interventions may include return to work or school coordination, applications for programs or services, assistance with completing FMLA or disability forms, coordination of follow up services, and interaction with governmental agencies when required.
    • Position requires concurrent chart audits for timely and effective documentation as well as coordination with utilization review departments for insurance reviews.
    • Assist management as needed.

    Pay: $22.00 - $25.00 per hour

    Expected hours: No more than 20 per week
