Case Manager, Sr. coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager, Sr. evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager, Sr. identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). The Case Manager, Sr. contributes to moderately complex aspects of a project. Work is generally independent and collaborative in nature. POSITION SUMMARY:
The Case Manager will serve and provide support to clients on a daily basis. This person should be focused and motivated, while working with facility management to handle client-related issues.

RESPONSIBILITIES
· Plan monthly in-house and offsite activities for clients.
· Transport clients to and from scheduled appointments.
· Supports and assist individuals in developing independent living skills and meeting rehabilitation goals.
· Conduct group and 1x1 sessions with clients.
· Oversee NSP resident goal, placement, and discharge.
· Meet with county case manager and conservators as needed.
QUALIFICATIONs
· Bachelor’s degree preferred.
· High School Diploma or GED.
· Experience working in the adult residential care industry preferred.
· At least two (2) year of Mental Health experience preferred.
· Knowledge of psychiatric illness and treatment.
· Ability to work independently and be self-motivated.
· Excellent interpersonal skills with the ability to effectively work with individuals and groups.
· Ability to flex work hours as needed.
· Willing to work some weekends and overnight shift.
· Possess strong interpersonal leadership, analytic, and problem-solving skills.
· CCL approved FBI and DOJ criminal background clearances.
· Valid driver’s license.
· CPR/First Aid/BLS certification
PHYSICAL REQUIREMENTS
· Ability to climb stairs multiple times a day and be able to lift and/or carry up to 25 pounds.
· Ability to sit, rise, kneel, squat, twist, pull bend multiple times a day.
· Ability to walk with items in hand, grasp and reach as needed.
· Ability to perform with repetitive maneuvers and manual dexterity.
· Ability to work in an office environment.
