Position Overview:
We seek a compassionate and experienced Client Care Coordinator, an RN, to join our team in Bismarck, ND. The Client Care Coordinator will be responsible for overseeing the care of our clients, who receive 24/7 care in the Bismarck area. This role will involve conducting client assessments, developing care plans, managing medication fills, training care professionals, conducting quality assurance visits, and collaborating with case managers to ensure the highest level of care is provided.
Qualifications:
- Bachelor's degree in nursing
- Previous experience in home care, healthcare, or a related field required
- Strong assessment and care planning skills
- Knowledge of medication management practices and procedures
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
