Clinical Pharmacist directs and develops research of existing and new prescription drugs and conducts utilization reviews to develop guidelines for usage, prior authorization, and updates to formulary. Monitors patient data on drug therapies outcomes, reactions, and errors. Being a Clinical Pharmacist provides education and outreach to medical staff on prescribing guidelines, alternative drug therapies, and risk mitigation. Administers a prior authorization program. Additionally, Clinical Pharmacist requires a doctorate degree in pharmacy (Pharm.D.). Typically reports to a manager or supervisor. Requires Registered Pharmacist (RPh).   • Position Summary:

    Under the leadership of the Director, Pharmacy, the Pharmacist is an active member of the department that assists the Pharmacy staff in the preparation and distribution of pharmaceutical products. The Pharmacist evaluates medication orders and determines the appropriateness of those orders.  The Pharmacist evaluates and consults with the medical and nursing staff on the patient’s responses to medication therapies and implements positive pharmaceutical solutions based on the age, sex, race and current medical condition of the patient. The Pharmacist prepares and provides medication, drug information and pharmaceutical services to patients, nurses, physicians and other hospital personnel.

     

    Basic Qualifications:

    • Education:

    Requires the completion of an accredited PharmD. Program.

    • Experience:

    Requires up to four years of work-related experience or any equivalent combination of education, training and experience.

    • Licensure, Registrations & Certifications:

    Requires and maintains a Pharmacist license..

    Requires and maintains a valid driver’s license and proof of insurance.

    Motor vehicle report required upon hire and reviewed annually.

     

    Essential Job Responsibilities:

    • Evaluates medication orders, determines the appropriateness of those orders and implements positive pharmaceutical solutions based on the age, sex, race and current medical condition of the patient.
    • Provides medication, drug information & pharmaceutical services to patients, nurses, physician & other hospital personnel.
    • Consults and evaluates the patient’s responses to medication therapies with the medical and nursing staffs and recommends alternative medications as required.
    • Maintains methods and systems for the acquisition, delivery, storage and handling of pharmaceutical products and the delivery of pharmaceutical services to the customer.
    • Prepares intravenous solutions and admixtures using aseptic technique.
    • Maintains computer skills adequate to comprehend and operate the Meditech System.
    • Ensures that medication administration is documented into the Meditech System, assuring patient charges are generated.
    • Provides medication product information to patients, staff and the community.
    • Mentors Pharmacy Technicians and students.
    • Transports drugs and medications to the Nursing Home or other off site locations, picks up or returns drugs or medications from supplier locations as needed .
    • Functions as part of an interdisciplinary team promoting positive interpersonal communication skills when communicating with patients, families, visitors, physicians, and staff in other departments of the organization.
    • Complies with CRHS and department safety, infection control and performance improvement initiatives and sterile procedures, ensuring the delivery of products and services.
    • Presents a professional image to CRHS nursing and medical staff members.
    • Performs other related job duties as assigned.

     

    Standards of Performance:

    • Provides medication, drug information & pharmaceutical services to patients, nurses, physician & other hospital personnel.
    • Prepares pharmaceuticals for patients that are age appropriate with consistent dosage variations for newborn, pediatric, adolescent, adult and geriatric patients.
    • Prepares intravenous solutions and admixtures using aseptic techniques.
    • Maintains inventory of supplies and reorders them accordingly.
    • Assists in filing prescriptions using unit dose medications and prepares or compounds pharmaceutical dosage as indicated by the prescriptions.
    • Complies with all standards of care, safety and infection control policies and procedures.
    • Demonstrates reliability as measured by compliance with the CRHS Time and Attendance policy.
    • Demonstrates professional conduct and adherence to all safety, confidentiality and HIPAA standards.
    • Promotes teamwork with the entire staff as well as other contacts within the hospital, as measured by observations of Manager and co-workers.
    • Attends professional development programs and in-service training and meets the minimum requirement for continuing professional education.
    • Adheres to the CRHS Standards of Behavior as measured by observations of staff, as well as outside and other inside customers.
    • Verbalizes all core competencies of the unit and department.
    • Complies with all regulatory requirements including the National Patient Safety Goals as observed by peers, supervisor and all members of the healthcare team.
