Clinical Pharmacist directs and develops research of existing and new prescription drugs and conducts utilization reviews to develop guidelines for usage, prior authorization, and updates to formulary. Monitors patient data on drug therapies outcomes, reactions, and errors. Being a Clinical Pharmacist provides education and outreach to medical staff on prescribing guidelines, alternative drug therapies, and risk mitigation. Administers a prior authorization program. Additionally, Clinical Pharmacist requires a doctorate degree in pharmacy (Pharm.D.). Typically reports to a manager or supervisor. Requires Registered Pharmacist (RPh). This full clinical service program will operate through a Collaborative Practice Agreement with independent medical facilities in the Rockingham, NC area.As a Clinical Pharmacist, you will establish and maintain professional relationships with all team members within the medical practice. Responsibilities may vary depending on the unique needs of each independent practice, offering you a diverse and stimulating workload. Must live in a commutable distance to the posted job location – this role is performed in provider clinics and may require regional travel to a group of clinics.
In this role, you will have the opportunity to:- Build and work directly with your patient panel to drive clinical outcomes and results
- Spearhead patient enrollment, device management, and data interpretation for RPM
- Collaborate on medication management with supervising providers
- Participate and lead topic discussions for current staff and residents
- Precept PGY-1 Students and participate in In-Clinic Rx Fellowship Program
- Develop individualize care plans based on specific patient needs
- Provide recommendations or implement therapy changes, when necessary
Must Haves:- A current, valid license to practice pharmacy in North Carolina.
- Current CPR certification.
- Certification for Immunization.
Please note, this career opportunity is open to all experience levels. Retail pharmacist and current clinical pharmacists are all encouraged to apply though preferred candidates would also have one of the following:- PharmD Degree with three years of experience
- Currently hold a Board of Pharmaceutical Specialty Certification or Completion of APhA/ASHP Accredited PGY-1 or PGY-2 Residency Program.
