Mountain Laurel Integrated Healthcare (MLIH) is seeking a Case Manager for our ACT (Assertive Community Treatment) Program. This position will work primarily between Logan and Mingo Counites in West Virginia. ACT Case Manager serves a population of chronic mentally ill, some of whom have dual diagnoses, by providing the basic core services of case management (including service planning, linkage, referral, assessment, advocacy, and crisis support as needed).
Minimum Requirements: Bachelor’s degree in a human services field.
