    AGENCY DESCRIPTION

    One of the oldest organizations of its kind in the country, Community Partners in Action (CPA) was founded nearly 150 years ago to champion criminal justice reform and advocate for preserving the dignity of those in and out of prison. Through our programs that include reentry and housing, youth initiatives, a nationally recognized Prison Arts Program, and holistic alternatives to incarceration, we provide a long-term impact that positively transforms individuals and society at large. Annually serving nearly 7,000 individuals throughout Connecticut, our work is possible due to partnerships with and support from individuals, government, private funders, organizations, and businesses.

    CPA launched our BEDI (Belonging, Equity, Diversity, and Inclusion) Journey in June 2020.     PROGRAM DESCRIPTION

    The Hartford Reentry Welcome Center Case Manager supports participants returning home from prison to Hartford and the surrounding areas with their successful reintegration back into their communities. The Case Manager provides strategic case management services and assists participants with accessing the community resources needed post release. Services and resources include but are not limited to:

    • Living wage and long-term employment assistance
    • Access to housing
    • Access to primary care and substance use and mental health services
    • Access to transportation
    • Gender responsive reentry and recovery programs

    POSITION REQUIRMENTS

    Bachelor’s, preferably in Criminal Justice or Social Services and at least two years of experience providing case management services. Experience may be considered in lieu of education. Must have strong emotional intelligence and written and verbal communication skills; knowledge of community providers that offer services to the criminal justice population; ability to interact with a multi-cultural population and crisis management skills. Decision- making and knowledge of cognitive-behavioral approaches are important. A valid driver’s license and reliable transportation are required. Bi-lingual capacity (English/Spanish) preferred. A candidate in recovery for at least five years and/or formerly incarcerated is desirable. If formerly incarcerated, the individual must have completed parole and/or probation and have no criminal justice involvement for at least three years.

    ESSENTIAL FUNCTIONS AND BASIC DUTIES:

    1. Commit to Reentry Welcome Center mission and goals.
    2. Provide long-term case management services to participants for at least up to 12 months in the community.
    3. Work in collaboration with the program’s Reentry Navigators.
    4. Assist each participant to increase their knowledge and understanding of behavioral patterns to promote change and help end the cycle of recidivism.
    5. Conduct participant risk/needs assessments using motivational enhancement strategies.
    6. Utilize the outputs of the assessments to formulate participants’ Individual Service Plans (ISPs).
    7. Make appropriate participant referrals based on individual needs.
    8. Collaborate with participants’ parole or probation officers to ensure post release compliance.
    9. Guide program participants to develop concrete goals including but not limited to housing, substance abuse, mental health treatment, educational, vocational and employment.
    10. Develop and maintain a strong network of community providers that can assist program participants.
    11. Participate in community groups, committees, and meetings.
    12. Educate, advocate, and promote social justice and social change on behalf of all program participants.
    13. Submit program reports to supervisor as prescribed.
    14. Collaborate with CT Department of Correction staff both in the facility and in the community.
    15. Function as a team-player assisting with all program participants and staff.
    16. Serve as a role model to participants and staff modeling pro-social behaviors.
    17. Establish and maintain professional boundaries with participants and community providers.
    18. Other duties as required for smooth operation of the program.
