Gateways Community Services is seeking a Service Coordinator to join our collaborative, interdisciplinary team of professionals with experience working with individuals with developmental/intellectual disabilities. Gateways Community Services believes that all people are of great value and strives to be innovative when providing quality supports needed for individuals to lead meaningful lives in their community. We support a diverse population of infants, children, and adults with developmental and intellectual disabilities. We are seeking a caring individual, dedicated to advocating and improving the lives of others. In this position, the Service Coordinator will be leading the collaborative team of stakeholders involved in providing the highest quality supports so our adult participants may lead meaningful lives in their community. Essential Job Responsibilities: Service Coordination / Case Management: Facilitate the annual service planning process, utilizing person-centered planning tools, techniques and SMART goals Provide assistance for individuals to access support services dependent on their needs (i.e. residential/vocational, medical, dental, psychological services, adaptive equipment, etc.)Information and referral to generic and specialized community supports Facilitate communication and maintain effective professional relationships with the team Maintain a working knowledge of adhere to all applicable state and federal program regulations Identify and advocate for needed funding for services Exercise discretion and independent judgment regarding development and monitoring of all services Manage Assistance to individuals' funds Negotiate service contracts with responding vendors and acquire services that are quality and value based.
