  • Community Action Partnerships of Western Nebraska (CAPWN) is a non-profit community-based health and human services organization that serves low-income, disadvantaged and those unable to meet their needs through other sources. The agency was established in 1965 and provides services to over 9,000 individuals, children, and families in three main areas:

    • Community Health Services: WIC, Commodity Supplemental Food, Foster Grandparents, Migrant Seasonal Head Start, and Child Development Center.
    • Clinical Health Services: Primary Care, Reproductive Health, Diabetes Management, Immunizations, Migrant Health, Ryan White HIV; Behavioral Health including Counseling Services, Medication Management, Medication Assisted Treatment and Substance Use Counseling and Intensive Outpatient Services and Dental Services.
    • Supportive Health Services: Family Stabilization, a Teen Outreach Program, SOAR, Youth Leadership Program, Youth Shelter, and Housing Assistance Programs.

    Job Title: Housing Case Manager
    Reports To: Housing First Manager

    Areas of Responsibility
    : Under the supervision of the Housing First Manager, this position is responsible for the implementation and delivery of comprehensive case management and advocacy services for individuals 25 years and older and their families participating in Transitional and Rapid Re-Housing through the Rural Set Aside Program (RSA). This position provides services based on the Housing First Model of care with motivational interviewing, strengths-based goal setting, and trauma-informed care approach to work collaboratively with individuals and families towards the general goals of maintaining housing, increasing family stability, in providing family functioning and increasing the self-sufficiency in the program.

    Primary Duties
    :
    • Conduct the VI-SPDAT with individuals who are experiencing homelessness.
    • Receive referrals from community queue and verify household meets criteria for program.
    • Intake and assessment of referred individuals and families for RSA Program.
    • Maintain updated knowledge and offer individuals emotional support, information about community resources and services (including employment, housing, shelter services, health care, etc.), and assistance completing paperwork (social services applications, legal filings, etc.)
    • Maintain a caseload of approximately 10 individuals and families.
    • Work to secure rental agreements with landlords and assure compliance with HUD requirements.
    • Conduct ongoing, individualized case-management to individuals and families in the program.
    • Assist individuals in developing and implementing an individualized program plan to meet long-term housing goals.
    • Develop and facilitate support, informational classes/sessions, life skills, and/or group activities designed to help clients explore and resolve self-care issues that may be influencing the client’s progress towards meeting their established goals.
    • Maintain accurate and confidential case files.
    • Do statistical data entry, complete routine paperwork.
    • Develop relationships within landlords and property management companies.
    • Attend and participate in staff meetings and training as requested.

    • Perform other duties as assigned.

    Working Conditions:
    • Includes office setting, off-site home visits, training, meetings and community interactions.
    • Work requires the ability to meet tight timelines.
    • Work receives limited supervision and requires the ability to take initiative and be self-directed.
    • Occasional out-of-town travel for training.

    Physical Requirements:
    • Must be able to move about quickly if necessary.
    • Must be able up to 50lbs safely.
    • Physically able to perform job.
    • Must complete health physical upon entry to program and yearly health screenings.
    • Ability to bend, stoop, sit, stand, squat, crouch and pull file drawers.

    Minimum Qualifications
    :

    Grade: 12
    • High School education or equivalent.
    • Two years of documented work experience in a related field preferred.
    • Experience with shelter programs homeless shelters, substance abuse programs, social services programs preferred.
    • Incentive for Spanish speaking.
    • Knowledge of “Housing First” approach to end homelessness preferred.
    • Knowledge or understanding of fair housing, tenant/landlord rights and responsibilities preferred.
    • Ability to communicate effectively both orally and in writing and good organizational skills.
    • Able to attend meetings and training outside of the immediate community.
    • Must be able to pass all onboarding requirements which include but are not limited to Adult/Child Protective Services, criminal and sex offender background checks.
    • Must have a valid driver’s license with good driving record and proof of insured vehicle.
    • Age 21 or above

    Grade: 13
    • Must meet minimum requirements above AND;
    • AA Degree in a related field with at least 1 years’ experience OR;
    • A minimum of 3 years’ experience in a similar position.

    Grade: 14
    • Must meet minimum requirements above AND;
    • BA degree in a related field OR;
    • A minimum of 5 years’ experience in a similar position.
