    CCAP Mission Statement - To empower all people and communities, challenged by poverty and social and cultural barriers, through advocacy, education, and access to high-quality health and human services.

      

    The OneCranston Health Equity Zone (HEZ), one of 15 HEZs in Rhode Island, is a resident-driven initiative funded by the Rhode Island Department of Health, with Comprehensive Community Action (CCAP) as our backbone agency. Our goal is to support and equip Cranston community members to collaborate and create projects, places, and programming where residents can healthily live, learn, work, and play.


    Located at the OneCranston HEZ’s Family Support Center, The Huddle Center, the part-time Family Engagement Specialist will be responsible as part of a team for the outreach and coordination of supports and activities for Cranston families with children. This position will include engaging families in the community, inviting them to our space and promoting positive connections. It will also include developing workshops and events, reinforcing effective practices and child development as well as promoting trust and support.


    20 hours/week. Some evenings and weekends required weekly to accommodate neighborhood family access.

    Requirements

    REQUIRED QUALIFICATIONS:

    • Minimum of AA degree in related field required, such as Human Services, Child & Family Development, Social Work, Education or Psychology. BA or BSW preferred. Equivalent, relevant work experience will be considered as substitute for education.
    • Previous experience in human services-related field, work with families and youth preferred.
    • Effective communication skills, time management and professionalism.
    • Knowledge and willingness to learn about Cranston and Rhode Island systems, benefits and supports for families.
    • Proven ability and demonstrated respect and sensitivity to provide linguistically and culturally relevant coordination and collaboration across systems and across diverse residents/communities.
    • Ability to utilize supervision and follow direction, working as part of a team.
    • Ability to act flexibly, accommodating unique family situations and scheduled activities.
    • Bilingual/bicultural, representative of target population a plus.


    WORK SCHEDULE DEMANDS:

    • May be required to work at any CCAP component and will be required to work in the community.
    • Due to the nature of the work and level of responsibility, this individual maintains a flexible schedule, which may go beyond a regular workweek.


    CONFIDENTIALITY OF INFORMATION:

    Has limited access to clinical records assigned to caseload.  Must follow State and Federal Laws of confidentiality of Clinical Records and must follow CCAP policies and procedures for the creation, protection, retention, release and disposal of Clinical Records.


    KEY RESPONSIBILITIES:

    • Engage families with children, assessing their needs and interests, and linking them to services/resources that might address those needs.
    • Outreach to the Cranston community, promoting the availability of services and making important connections.
    • Facilitate child- and family-focused activities, including programs, workshops, events.
    • Maintain updated knowledge about both CCAP and community resources, as well as those responsible for      enrollment/information, allowing for warm hand offs as needed.
    • Actively participate in community planning and the development of a strong OneCranston HEZ membership and work plan addressing priority needs.
    • Other duties as assigned.

    While performing the duties of this job, the employee is regularly required to sit and to talk to hear.  The employee frequently is required to use hands, fingers, handle or feel objects, tools or controls and reach with hands and arms.  The employee is occasionally required to stand and walk.  The employee must occasionally lift and / or move up to 20 lbs.  Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.  The environment is a clinical business office environment.   Potential exposure to blood borne pathogens, respiratory diseases and other infectious diseases, and other biohazardous material requiring universal standard precautions.  This position requires the individual to have their own transportation, a current unrestricted license to drive, documented auto insurance and the individual must be willing to work in schools, health offices and the community.
