Manages specific community health programs from planning and design through implementation and evaluation. Additionally, Community Health Director requires a master's degree in a related area. Typically reports to a top management. The Community Health Director manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. To be a Community Health Director typically requires 5+ years of managerial experience. Deep knowledge of the managed sub-function and solid knowledge of the overall departmental function. Position Responsibility: Plan, evaluate, implement, develop, and direct the overall operations of nursing services across agency programs in accordance with current federal, state, and local standards, guidelines and regulations that govern the agency to ensure the highest degree of quality care is maintained. Assist in the design, development, and implementation of health-related community-based initiatives and programs.
This includes responsibility for Personal Care service through Home Care as well as programs with a significant health component such as Enhanced Community Options Program, Clinical Eligibility and Assessment Services, HCBS Waiver, Personal Care Attendant, and Accountable Care Organizations. The Director of Nursing and Community Heath Programs promotes an interdisciplinary approach to assure that consumers receive appropriate service plans and interventions.
