Typically reports to a top management. The Community Health Director manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. To be a Community Health Director typically requires 5+ years of managerial experience. Deep knowledge of the managed sub-function and solid knowledge of the overall departmental function.     Job Title: | Promotor/a - Community Health Worker
    FLSA Classification: Non- Exempt
    Department: | Diabetes Health Center
    Compensation: | $23-$30
    Reports to: | Program Manager – Clinical Services 

    Summary:
    As a part of the care team, the Promotora/Community Health Worker is responsible for improving health outcomes by providing health promotion, disease management, and health education services to diverse populations and supporting community members and their social and health needs by referring them to community resources, services, and medical systems. 

    Essential Duties and Responsibilities:
    • Provide patient-centered health coaching and education activities across the continuum of care to facilitate high-quality, cost-effective health promotion programs designed to improve health outcomes.
    • Act as a liaison between the patient, the patient’s support network, treating physician(s), and ancillary providers.
    • Work collaboratively with all CHT team members and stakeholders utilizing a multi-disciplinary approach to assess, identify, plan, set goals, and measure outcomes.
    • Assess social determinants and health, needs, and assets of priority populations to determine client needs and make referrals to family support, health, and social service agencies in the area, following up with individuals to ensure they receive needed services.
    • Act as a patient advocate working to resolve patient concerns and barriers to access healthcare and wellness services.
    • Develop relationships with local health care and social service providers to facilitate information sharing and service provision.
    • Actively engage and build rapport with community members by participating in public/community events.
    • Attend and participate in assigned organizational meetings and activities, including special events.
    • Complete other tasks and duties as assigned in support of the Health Trust mission.

    Supervisory Responsibilities: None.

    Knowledge, Skills, and Experience:
    • A high school diploma/GED is required.
    • Bilingual (Spanish and English) required.
    • A valid CA Driver’s License is required.
    • Bachelor’s degree in Social Work, Psychology, Public Health, Nursing, Community Health, or Health-related field preferred, OR equivalent experience is a plus but not required.
    • Excellent time management and organizational skills.
    • Excellent written and verbal communication skills must be able to communicate clearly and concisely with all levels of individuals, sometimes in stressful situations.
    • Ability to tailor information at the appropriate level of health literacy and to interact respectfully with diverse cultural and socio-economic populations.
    • Must be able to work flexible hours including evenings and weekends as needed.
    • Must be able to manage stress and multiple simultaneous work demands.
    • Must demonstrate respect, concern, and empathy for the spiritual and emotional needs of patients.
    • Ability to work independently, problem solve, and design creative solutions to complex challenges.
    • Detail-oriented, organized, and efficient.
    • Strong proficiency in Windows-based database applications.
    • Naturally friendly, punctual, professional self-starter who enjoys being part of a team that thrives on helping others to support local healthcare delivery.

    • Continuously (67-100%): use hands to finger, handle or feel objects, tools, or controls; see (including close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus), hear, and speak; key (i.e., computer, calculator, telephone); and lift (up to 5 lbs); use writing instruments, computer.

    Work Environment:
    • Approximately 50% of the time performing job duties is spent indoors, within a standard office or clinic environment.
    • Approximately 30%-70% of the time is spent on the computer.
    • Approximately 75% of the time is spent interacting with colleagues and members of the community.
    • The noise level in the work environment is usually moderate.
    • The temperature of the work environment is moderate and ranges from 65 – 80 degrees.

    Travel Requirements: This job requires regular local travel (50%).
