POSITION SUMMARY
The Director of Community Health at BJHCHS is responsible for overseeing and coordinating all community health and outreach activities within the organization. This position requires a Bachelor's degree in Social Work, Public Health, or a related field, with a preference for candidates holding a Master's degree in the same fields and Community Health Worker certification. The successful candidate will have a minimum of 3 years of experience in public health, community medicine, outreach, or the medical field. The Director will lead a team of Community Health Workers, Outreach Workers, and Social Workers, providing supervision and guidance. Applying a population health perspective, the Director will play a crucial role in improving the overall health and well-being of the communities served by BJHCHS.
RESPONSIBILITIES
- Leadership and Team Management:
- Supervise and provide leadership and mentorship to a team of Community Health Workers, Outreach Workers, and Social Workers.
- Develop and implement performance goals and objectives for the team and conduct regular performance evaluations.
- Provide guidance, support, and training to ensure the team's effectiveness and professional growth.
- Foster a collaborative and inclusive work environment that promotes teamwork and individual development.
- Community Health and Outreach Coordination:
- Develop and implement strategic plans for community health and outreach initiatives, in alignment with the organization's mission and goals.
- Coordinate and oversee all community health and outreach activities, ensuring their successful execution.
- Collaborate with internal departments and external partners to identify community health needs and develop programs to address those needs.
- Monitor and evaluate the effectiveness of community health and outreach programs and make recommendations for improvements.
- Program Development and Evaluation:
- Conduct needs assessments to identify gaps in community needs under the direction of the COO and other Senior leadership.
- Design, implement, and evaluate evidence-based programs and interventions to address identified needs.
- Ensure compliance with relevant regulations, guidelines, and funding requirements.
- Establish and maintain partnerships with community organizations, healthcare providers, and other stakeholders to enhance program effectiveness and sustainability.
- Data Management and Reporting:
- Develop and maintain systems for collecting, analyzing, and reporting data related to community health and outreach activities.
- Prepare reports on program outcomes, trends, and impact, and present findings to internal and external stakeholders.
- Utilize data to identify areas for improvement and guide decision-making processes.
- Budgeting and Resource Management:
- Participate in the development of annual budgets for community health and outreach programs.
- Monitor expenditures and ensure effective and efficient use of allocated resources.
- Seek additional funding opportunities through grants, partnerships, or other means to expand and enhance community health and outreach initiatives.
- Addressing Social Drivers of Health:
- Recognize and understand the impact of social determinants of health on community well-being, such as socioeconomic status, education, housing, and access to healthcare.
- Ensure that community health and outreach programs incorporate interventions that address the underlying social determinants of health.
- Community Engagement, Collaboration and Partnerships:
- Foster meaningful community engagement and participation in the planning, implementation, and evaluation of community health programs.
- Establish and maintain relationships with community organizations, leaders, and residents to gain insight into community needs and priorities.
- Actively participate in community coalitions, task forces, and committees focused on addressing community health needs.
- Seek opportunities for partnerships and joint initiatives to leverage resources, expertise, and reach maximum community impact.
- Health Education and Promotion:
- Develop and implement health education initiatives that empower individuals and communities to make informed decisions regarding their health.
- Coordinate and deliver educational workshops, seminars, and outreach activities that promote preventive care, healthy lifestyles, and disease management.
- Collaborate with the Operations team to develop culturally appropriate health promotion materials and campaigns.
The Director of Community Health and Outreach plays a vital role in promoting the health of the communities served by BJHCHS. By coordinating and leading community health initiatives, the Director will contribute to creating a healthier, more equitable community by addressing the underlying factors that impact health outcomes. This comprehensive approach will improve the overall well-being and quality of life for individuals and populations served by BJHCHS.
QUALIFICATIONS
- Bachelor’s degree in social work, Public Health, or a related field (Master's degree and certification as CWH preferred).
- Minimum of 3 years of experience in public health, community medicine, or outreach
- Certification as a CHW and CHW Supervisor must be obtained within the first 6 months if not already completed
- Strong leadership and management skills, with experience in supervising a diverse team.
- Knowledge of population health principles and strategies.
- Excellent communication, interpersonal, and collaboration skills.
- Ability to develop and maintain partnerships with diverse stakeholders.
- Proficiency in data management and analysis.
- Understanding of relevant regulations and guidelines in community health and outreach.
