Community Health Connections (CHC) is a multi-site, non-profit health care center offering urgent care, primary family medical and pediatric care, preventative and restorative dental care, oral surgery, behavioral health services for children and adults, and substance use disorder treatment, and specialty services including optometry eye care, optical shop, acupuncture, nutrition consultations and podiatry. CHC has five sites within Fitchburg, Gardner and Leominster with decades of experience as a Federally Qualified Health Center (FQHC), serving 36 communities in North Central Massachusetts.

Under the direction of the Director of Medical Operation, the Community Health Worker provides outreach and care coordination to the identified population and coordinate administrative requirements per the contact with the Connecticut River Valley Farmworker Health Program (CRVFHP). Activities will include direct outreach to the target population and local farm owner/operators to ensure access to health care, enrollment into the program per guidelines set forth by CRVFHP, coordination with outside specialty providers to ensure access and proper coordination of benefits, and regular data gathering and reporting as required by the program.

Essential Duties and Major responsibilities:

• Provide outreach to migratory and seasonal agricultural workers (MSAW) at local farms to educate about the available services
• Register MSAW in the applicable EHR including ensuring and assisting patients in completing applicable new patient forms
  and program eligibility forms
• Schedule appointments for health care services at the main health center locations as needed
• Tracking of outreach hours and activities
• Travel to farms for clinic service clinics; delivering medication, supplies (such as masks, home COVID tests, etc.)
• Performs Medical Assistant tasks during on-farm clinics
• Provide education to MSAWs regarding health-related topics at the direction of clinical leadership
• Complete required CRVFHP data collection including but limited to: online eligibility forms, annual demographic reporting and
  program utilization
• Become a Certified Application Counselor and assist patients in applying for Medicaid benefits as appropriate
• Assist in coordinating needed services with outside providers, i.e. pharmacies and specialists to ensure proper reimbursement
  under the program guidelines
• Participate in CRVFHP meetings as scheduled

Minimum Qualifications:

• High School Diploma required
• Clean driving record (for authorized operation of company mobile unit)
• 5 years experience in healthcare or program coordinator preferred
• Experience as a Medical Assistant, Nursing Assistant or similar a plus
