Targeted Case Manager (TCM)

How you can help as a Targeted Case Manager (TCM): Case managers responsible for assisting the person and the persons support network to identify, select, obtain, coordinate, and use both paid services and natural supports that may be available to enhance the person's independence, integration, and productivity consistent with the person's capabilities and preferences as outlined in the person's person-centered support plan.

Pay Rate: $18 per hour

Schedule: M-F 8:00AM-4:30PM

Requirements for the Targeted Case Manager (TCM) role:

• Must have Bachelor's degree in Human Services and 6 months full-time experience required. Five years related/verifiable full-time experience in the field of developmental disability in lieu of degree.
• Be at least 18 years old
• Valid Driver's License for a minimum of 2 years with a good driving record.
• Pass drug and alcohol screenings
• Pass criminal background checks
• Must be available for 2-4 weeks of PAID training, M-F 8:30am-5:00pm.
• ASL (American Sign Language) preferred
