Collects patient data to monitor health status and proactively manage the patient's condition. Being a Diabetes Educator facilitates skill development and behavior modification so that the patient is able to successfully manage the disease. Typically requires a bachelor's degree in nursing or equivalent. Additionally, Diabetes Educator typically required to be a Certified Care and Education Specialist (CDCES). Typically reports to a manager or head of a unit/department. The Diabetes Educator occasionally directed in several aspects of the work. Position Summary : The Diabetes Educator is integral to the development and improvement of patient care that affect inpatient and outpatient diabetes management and nutritional therapy needs. Under general supervision, provides accessible and therapeutic learning experience for people with diabetes and/or nutritional needs. The Diabetes Educator is responsible for overseeing the day-to-day operations of the Diabetes Self-Management Education and Support (DSMES) program and ensures that the National Standards for Diabetes Self-Management Education and Support (NSDSMES) are met and maintained at all times.
