Diabetes Educator educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Collects patient data to monitor health status and proactively manage the patient's condition. Being a Diabetes Educator facilitates skill development and behavior modification so that the patient is able to successfully manage the disease. Typically requires a bachelor's degree in nursing or equivalent. Additionally, Diabetes Educator typically required to be a Certified Care and Education Specialist (CDCES). Typically reports to a manager or head of a unit/department. The Diabetes Educator occasionally directed in several aspects of the work. Requirements for consideration:
- Bachelor's degree
- 1 year RD experience or RD work /internship experience; or 250 hours of DSME/T related work experience.
- Registered Dietitian certificate upon start
- Basic Life Support required Upon Start from AHA and certified in CPR every other year
- Strong Diabetes education background strongly preferred.
Job Responsibilities:
- The Diabetes Nutrition Educator assesses the needs of diabetic patients and design, implements and evaluates a teaching plan for each patient.
- Provides individualized care for all aspects of medical nutrition therapy (MNT) and diabetes self-management for individuals and groups.
- Provides leadership to others regarding practice and serving as a resource, preceptor and mentor.
- Plans, develops and implements new programs to meet the needs of the patient population and enhance diabetes self-management education within Inova Health System.
- Utilizes the unique contributions of each discipline and applying an integrated team approach to improve patient outcomes.
- Performs other duties as assigned.
