Position Description:
A Disability Services Case Manager provides person-centered assessment, planning, facilitation and advocacy for options and services to meet the needs of elderly or adults with disabilities who are enrolled on Community Access for Disability Inclusion (CADI) Waiver program in Ramsey County.
This position will monitor health and safety needs of individuals and intervene as necessary, ensure effectiveness of services provided and collaborate with other professionals working with those you serve.
