We provide exceptional quality care management and guardianship to the frail elderly and people with disabilities throughout the Chicago Area. Our services are paid privately by trusts, estates, and families. The Care Management Team at Midwest performs duties for the full life cycle of clients, ranging from care planning for special needs children to geriatric care management and end-of-life planning. Duties include conducting assessments, care planning, advocacy and acting as a liaison between involved parties. We are professional, agile, and supportive.

    Ideal candidates are highly professional, detail-oriented, have strong written and verbal communication skills, strong conflict management skills, are self-directed, and are motivated by helping people achieve their highest quality of life. Some weekend and evening hours will be required to meet client needs.

    We are currently recruiting for team members with 3 years of professional experience in our field. We service the greater Chicagoland area and have a need for employees who can serve the southern Cook County region.

    The duties and responsibilities of the Care Manager are:

    • Visiting with clients in their homes, nursing homes, or other placements
    • Developing relationships with clients and their family systems
    • Attending medical appointments and care conferences
    • Creating and implementing care plans
    • Making medical decisions, including end-of-life planning
    • Shopping and buying everyday supplies such as medications, groceries, and/or clothes
    • Performing comprehensive psycho-social and health assessments
    • Creating and adhering to client budgets
    • Placing and supervising caregivers through agencies
    • Interfacing with medical and allied health professionals
    • Communicating with families, attorneys, and trust officers
    • Updating client records; Tracking and entering time and expenses
    • Attending court to report on progress of care plan
    • Special Projects as designated by Supervisors

    Qualifications and experience:

    • Master's Degree Preferred; Bachelor’s Degree with 3 years of experience in a health care related field or social services will be considered.
    • Must have experience working directly with at least one client population (elderly, individuals with mental illness, people with disabilities).
    • Must possess good clinical skills and the ability to assess a situation/client, make recommendations for changes, and implement recommendations.
    • Must have the ability to multi-task, successfully manage conflict, and communicate clearly.
    • Must have a vehicle with insurance.
    • Must have reliable home internet.

    Position structure

    • The Care Manager will travel to visit clients and attend meetings. The Care Manager will spend most of their days in the field.
    • The Care Manager will often begin and end their day at their home and will need a place to make phone calls, write emails, and do some paperwork.
    • The Care Manager will work in the office every day during orientation; Care Managers report to the office 1-2 days per week after onboarding is complete.
