```Job Summary:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing comprehensive support and guidance to individuals and families in need. Your role will involve assessing client needs, developing care plans, coordinating services, and advocating for clients within the community. The ideal candidate will have a strong background in social work, crisis intervention, and behavioral health.

Responsibilities:
- Conduct thorough assessments of client needs and develop individualized care plans
- Coordinate and monitor services to ensure clients receive appropriate support
- Advocate for clients' rights and access to resources within the community
- Provide crisis intervention and support during times of emotional distress
- Collaborate with other professionals, such as healthcare providers and social service agencies, to ensure holistic care for clients
- Monitor client progress and adjust care plans as needed
- Provide mentoring and guidance to clients in areas such as workforce development
- Utilize motivational interviewing techniques to empower clients and promote positive change

Skills:
- Strong knowledge of Medicare regulations and processes
- Experience in behavioral health assessment and treatment planning
- Understanding of crisis management techniques and interventions
- Proficient in conducting comprehensive assessments and developing care plans
- Excellent communication skills, both written and verbal
- Ability to work collaboratively with a multidisciplinary team
- Strong problem-solving skills and ability to think critically
- Empathy, compassion, and cultural sensitivity
