Disease Management Case Manager coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Acts as a liaison between patient/family, employer and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Disease Management Case Manager responsibilities include but are not limited to documenting case progress, identifying health risks, and reporting the findings of the case study at appropriate intervals. Requires an associate's degree/bachelor's degree, and is licensed to practice nursing. Additionally, Disease Management Case Manager typically reports to a supervisor or manager. Ascension DePaul Services of New Orleans (DCSNO) affiliates with DCHC which manages multiple Federally Qualified Health Center locations in the Greater New Orleans area. DCSNO is committed to improving the health status of communities and continues a long tradition of health ministry. DCSNO is seeking a full-time Infectious Disease Case Manager.
The Infectious Disease Case Manager will assist in the full implementation of social program needs with a special focus on resolving barriers to engagement in services, ensuring linkage to medical and behavioral health care, with overall goal of increasing client’s ability to self-manage, while facilitating integration of care for those individuals and families working with a multiple services systems. Case Manager will assess all new and existing clients for program eligibility, answer and respond to inquiry calls, and provide program information and referrals for new and existing clients.
