    Ascension DePaul Services of New Orleans (DCSNO) affiliates with DCHC which manages multiple Federally Qualified Health Center locations in the Greater New Orleans area. DCSNO is committed to improving the health status of communities and continues a long tradition of health ministry. DCSNO is seeking a full-time Infectious Disease Case Manager.

    The Infectious Disease Case Manager will assist in the full implementation of social program needs with a special focus on resolving barriers to engagement in services, ensuring linkage to medical and behavioral health care, with overall goal of increasing client’s ability to self-manage, while facilitating integration of care for those individuals and families working with a multiple services systems. Case Manager will assess all new and existing clients for program eligibility, answer and respond to inquiry calls, and provide program information and referrals for new and existing clients.


    DUTIES & RESPONSIBILITES

    • Provides advocacy and linkage to care that will help clients increase personal and family stability and achieve personalized care plan goals.
    • Assists in development, implementation and revision of individual treatment plans; assures that services provided are specified in the Treatment Plan and monitors progress toward treatment goals when necessary.
    • Serves as a liaison between the patient and the care team as appropriate.
    • Social, peer and community support.
    • Follow up on no show appointments to ensure clients are not lost to care.
    • Helping and empowering clients to develop and utilize independent living skills and strategies.
    • Receives documentation from providers and follows up with patients to assist in linking patients to resources.  
    • Reviews program eligibity with clients and assist with appropriate applications and outcomes.
    • Provides recommendations for appropriate grant assistance programs.
    • Sets up and attends meetings with providers and patients as needed.
    • Informs all parties involved on the processes that used to treat and/or link patients to services. 
    • Maintain patient confidentiality in treatment plans and reporting as appropriate. Understands and abides by HIPAA guidelines.
    • Communicates with patients regularly, via phone or in person to monitor their well-being, address their questions or concerns, and connect them with resources.
    • Provide for a face-to-face or phone contact, and a home visit, as determined by client needs.
    • Maintains patient records in accordance with applicable standards and regulations, grant requirements, etc.
    • Maintains a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature.
    • Enters applicable services in CareWare system.
    • Ability to perform Rapid HIV testing and counseling as needed at community events.
    • Attends meetings as appropriate and meets regularly with supervisor to exchange pertinent information and receive supervision.
    • Performs other duties as assigned.

    WORK ENVIRONMENT

    Work is normally performed in a clinic, office environment, school or mobile unit with varying temperatures.


    MINIMUM QUALIFICATIONS

    • Associate or Bachelor's degree preferably in a healthcare-related field
    • Two years of case management experience
    • Experience in collaboration with interdisciplinary healthcare
    • Good interpersonal and communication skills
    • Reliable Transportation (work would entail traveling to clinic locations and traveling to social service agencies in the community)
    • Flexible scheduling required


    PREFERRED QUALIFICATIONS

    • Experience in HIV/STI programs
    • Bilingual (English/Spanish) desirable
    • Knowledge of Ryan White programs
