Disease Management Case Manager coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Acts as a liaison between patient/family, employer and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Disease Management Case Manager responsibilities include but are not limited to documenting case progress, identifying health risks, and reporting the findings of the case study at appropriate intervals. Requires an associate's degree/bachelor's degree, and is licensed to practice nursing. Additionally, Disease Management Case Manager typically reports to a supervisor or manager. The Rare Disease Program Manager helps navigate and coordinate a patient needs through the complex medical system. This position partners with patients and families to ensure they understand follow-up needs and coordinates their medical appointments, as well as supports the families in a holistic approach and be a point of contact with complex needs. Rare Disease Program Manager Overview Key responsibilities include but are not limited to: Serves as a single point of contact for coordinating appointments, procedures, labs, imaging, or treatments Accurately navigates and translates complex service protocols, tools, standard work, and information to ensure appropriate services are delivered Gather and document information to help facilitate communication between the patient/caregiver and care team Maintains awareness of multiple competing situations needing attention and prioritizes to assure patient needs are met, while maintaining a pleasant, friendly demeanor.
