We are seeking a meticulous and motivated Performance Improvement / Quality Assurance Analyst to join our dynamic team. The ideal candidate will play a crucial role in reviewing services concurrently with retrospect in accordance with a written plan to determine that care is appropriate and consistent with the patient care plan, medical regimen, patient's needs, and meets the standards of compliance. Duties
- Evaluate documents ensuring a skill is documented with compliance of state and federal along with agency regulations.
- Helps perform required utilization reports.
- Provide leadership along with other management in promoting and maintaining standards for quality nursing care.
- Collaborate with teams to identify, document, and track defects using tools provided.
- Participate and focus on all disciplines towards patient progress / exacerbations and patient progress toward goals to ensure compatibility and performance.
Requirements
- Proficiency in home health with firsthand experience in Performance Improvement / Quality Assurance processes.
- Familiarity with Axxess Home Health software is a plus.
- Strong understanding of home health guidelines.
- Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) registration with the State Board of Nursing.
- Excellent analytical skills with attention to detail and problem-solving capabilities.
- Effective communication skills, both written and verbal, to convey findings clearly.
