Position Summary:
Health Affiliates Maine is seeking Veterans Community Engagement Coordinator to provide clerical/administrative support to our Veterans Case Management Program.
Roles and Responsibilities (include but are not limited to):
- Identify Key Partnerships to obtain Veterans case management referrals.
- Be able to engage Veterans at outreach events and accurately inform them about our service array.
- Research events and create a monthly engagement calendar of Veteran-centric events statewide and execute participation in these events (schedule staffing, work with vendors, prepare and set-up materials, provide outreach to designated population at the event).
- Nurture referral relationships by keeping referral sources updated about their referral status.
- Present to groups and individuals on the programs offered by HAM aligned with our marketing plan including providing education about our programs at webinars, outreach events, staff meetings and one-on-one engagements.
- Collect and input information on referral sources, outreach data and other key metrics for reporting on Veterans grants.
- Function as a representative for Health Affiliate Maine in meetings pertaining to the SSG Fox Grant and OBHS Veterans case management with partner organizations.
- Function as a SSG Fox program liaison with the community, helping to educate and solution-build to champion program access and participation.
- In collaboration with the Program Director and Coordinator, complete all required Veterans grant reporting.
- Assist the Program Coordinator with using squares system to screen for first line eligibility and entering eligibility information in the electronic health record.
Qualifications (include but are not limited to):
- MHRT/C, LSW or equivalent behavioral health background.
- Experience/skills in public speaking/presenting and outreach/education.
- Strong administrative and organizational skills.
- Experience with the military or veterans service program provision or lived experience preferred.
- Active Maine Driver's License, and ability to perform statewide travel.
