    JOB SUMMARY: Oversee and coordinate nursing care services and supports for the agency. Provide a mixture of supervision and oversight of agency nursing staff and direct nursing services, as well as initial and ongoing medical, health and safety training to staff.

    I. Key Result Areas/Accountabilities

    • Program Administration / Client Services
    • Supervise nursing staff and provide on-call support for departments as needed. Respond to emergency situations or provide crisis intervention when necessary.
    • Ensure required medical documentation is uploaded into Noble’s electronic database and the Bureau of Disability Services electronic portal within established timelines.
    • Provide Wellness Coordination for assigned clients, including, but not limited to: at least one visit per month to each client’s home to perform a nursing assessment, deliver medications, check documentation, etc.
    • Communicate with physicians and other medical providers of assigned clients as needed.
    • Attend physician and other medical appointments when needed as determined by the Community Living team.
    • Develop and monitor medication administration systems to ensure medications are administered as prescribed and properly documented.
    • Coordinate medication orders with physicians and pharmacy.
    • Develop, monitor, and update health/risk plans for individuals served and train staff accordingly.
    • Teach classes in Medication Administration, CPR, First Aid, Infection Control, Back Safety and other medical/safety subjects to staff as directed.
    • Screen questions and referrals concerning the health needs of individuals served and relay to physician when appropriate.
    • Provide counseling to individuals served and staff regarding nutrition, hygiene, medications, medical conditions, etc. as requested.
    • Train staff on individual-specific needs, such as g-tubes, blood sugar testing, and other medical interventions.
    • Answer calls and other communications from individuals served and staff regarding medical issues.
    • Seek out and/or act on opportunities to engage in advocacy efforts that preserve the rights of persons served.
    • Perform other duties and/or complete special projects as may be requested or assigned.
    • Communications Systems/Practices
    • Identify and utilize effective verbal, written and electronic communication skills and vehicles to accomplish tasks and clarify information for all internal and external customers.
    • Ensure that all communication takes place in a professional manner, reflecting dignity and respect for persons served.
    • Utilize people-first language in written and verbal communication.
    • Actively participate in department and committee meetings as appropriate, taking action on all assigned items.
    • Immediately alert appropriate supervisor of any potentially critical issue or crisis situation.
    • Quality/Service Assurance
    • Ensure that persons entering services have met medical clearance entry standards.
    • Continuously evaluate environment and procedures to minimize the risk of exposure to blood or other potentially infectious materials.
    • Monitor the safety of all persons served and make recommendations to improve safety practices.
    • Provide a schedule of training sessions for staff’s medical/safety training and renewal.
    • Promote health and safety of all staff and persons served through adherence to all agency policies and procedures.
    • Maintain all appropriate information in a confidential and professional manner, and in accordance with regulatory guidelines.
    • Adhere to the organization’s Code of Ethics and support the identified Position Statements.
    • Model appropriate behavior in interaction with persons served, staff, families, community resources and other audiences/customers.
    • Actively participate in personal professional development through use of Performance and Professional Development Plan, training opportunities and interaction with other staff and professionals.
    • Reporting
    • Maintain accurate health/medication/nursing records on persons served.
    • Utilize designated client software database to accurately and consistently document consumer information and services provided as necessary to meet agency requirements.
    • Prepare all necessary reports according to established timelines and in adherence to agency guidelines.
    • Industry Relations
    • Anticipate and respond positively to the needs of persons served, other providers, families, staff and other customers.
    • Maintain skill level, licensure and certifications in nursing, first aid, CPR and other areas as needed. Continue to build base of knowledge in the area of disabilities and related health issues.
    • Engage in leadership behavior to contribute to the organization’s growth and development and to the provision of individualized supports and services that facilitate personal outcomes for persons served.
    • Provide leadership/mentoring to others through example and the sharing of knowledge and skills, as appropriate and/or requested.
    • Develop and maintain professional relationships with all internal and external customers.
    • Network with agency and other professionals to develop and enhance knowledge, skills and abilities.

    Requirements:

    • Education/Experience
    • Must be a Registered Nurse licensed in the State of Indiana in good standing.
    • Minimum of one year of nursing experience. Experience working with individuals with Intellectual and Developmental Disabilities preferred.
    • Business/Technical Skills
    • Professional/technical knowledge of medical treatment, documentation, regulations and operating procedures.
    • Ability to remain calm in emergency situations.
    • Ability to learn and effectively utilize identified computer software applications to accomplish tasks.

    • General Requirements/Working Conditions
    • Must provide own transportation, have valid driver’s license and driving record must meet insurability requirements.
    • Must maintain and provide documentation of auto insurance at limits set by agency.
    • Must have ability to lift/move individuals or materials weighing up to 75 lbs. periodically.
    • Must have ability to stand, walk, bend and reach frequently.
    • Must maintain CPR and First Aid certification and must stay current on all other training as defined by state, accreditation body or organization guidelines.
