The impact of Child Crisis Arizona's work is immeasurable. This approach includes both crisis interventions, such as emergency shelter and family reunification services, as well as ongoing support to build family resiliency, including counseling, education, and parenting classes. ESSENTIAL FUNCTIONS

Establish trusting relationships with families quickly, while considering a families’ personal experiences and diverse cultural backgrounds.

Engage in face-to-face contact with community partners in hospital settings, clinics, and elsewhere, providing presentations and maintaining positive working relationships to foster further collaboration.

Promote community awareness regarding Healthy Families program services and developing relationships with community providers through community outreach activities in order to secure referrals.

Receive and process daily, referrals from hospital personnel as well as other agencies. Introduce the Healthy Families program directly to families in the Hospitals when possible.

Conduct enrollment visits in a family’s home, scheduled to meet the needs of the family

Compose accurate and complete in-depth screenings, assessments, program intake forms, and data tracking within appropriate timelines.

Provide comprehensive data, recording-keeping and reporting of community outreach efforts.

Adhere to HFA standards regarding contacts, time frames, assessment rates etc.

Complete all necessary reporting requirements

Offer and administer the FROG Assessment with all eligible families to determine risk level and need for services.

Attend and participate in required virtual and/or in-person trainings and program meetings.

Build and maintain relationships with community agencies and partners to share program information and learn about resources available for families.

Participate in Continuous Quality Improvement activities.

Supports the eligibility process for interested families including the collection and analysis of required documentation when necessary

Coordinates and participates in all family engagement activities such as parent meetings, workshops and events.

Completes additional assignments, tasks, and reports as directed
Education/ Experience:
-
Bachelors degree in related field or Associates Degree with early childhood experience

Bilingual English and Spanish required
-
Comfort, familiarity, or experience working in a hospital setting preferred.

Knowledge of family dynamics and community resources preferred.

Demonstrated attention to detail and follow through skills

Must be able to display verbal and written communication skills that meet the requirements of the position

Highly proficient with Microsoft Office Suite and comfortable with technology in performing job duties

Must be at least 21 years of age

Must be able to obtain Class One Fingerprint Clearance Card immediately upon hire (if there are any uncertainties whether an applicant may have to go through an appeal process: they are encouraged to complete that process prior to applying, as an invalid card will result in termination of employment)

Must have a clean 5 year driving record
