Location: Atlanta, TX, 75551
Skills: RN Field Case Manager
The RN Field Case Manager will be responsible for providing case management services to patients in their homes or other community settings. The successful candidate will work closely with physicians, nurses, and other healthcare professionals to ensure that patients receive the highest quality of care.
Responsibilities:
- Conduct initial assessments of patients' needs and develop individualized care plans
- Coordinate and monitor patient care, including medication management, wound care, and other treatments
- Provide education and support to patients and their families
- Collaborate with healthcare professionals to ensure that patients receive appropriate care
- Document patient progress and communicate with healthcare team members
- Ensure that patients receive timely and appropriate follow-up care
- Participate in quality improvement initiatives and other projects as assigned
Requirements:
- Current RN license in the state of Texas
- Minimum of 2 years of experience in case management
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
- Proficiency in Microsoft Office and electronic medical records
- Valid driver's license and reliable transportation

Field Case Manager
Works with the Interdisciplinary Group (IDG) to develop, coordinate, and update the individualized plan of care for hospice patients/families cooperatively with the patient and/or family, physician and hospice team.
