Our clinics provide a wide array of services to include in office outpatient counseling, school-based mental health, telehealth and day treatment for children and adolescents. Marriage & Family Health Services is currently seeking a Case Manager for our Marshfield office.

    We are seeking a candidate to enhance the quality of client management, maximize satisfaction and promote cost effectiveness. The successful candidate will be accountable for the full client care cycle. Case management duties will include assessing, planning, implementing, monitoring, and evaluating actions required to meet the client’s health and DHS 40 state requirements.

    Marriage & Family Health Services also offers regularly scheduled in-house professional development, career advancement opportunities, autonomy, but with access to regularly scheduled supervision with program clinical coordinator.

    Responsibilities

    • Coordinate and provide care that is safe, timely, effective, efficient, equitable, trauma-sensitive, and client-centered
    • Handle case assignments, draft service plans, review case progress and determine case closure
    • Help clients achieve wellness and autonomy
    • Facilitate multiple care aspects (case coordination, information sharing)
    • Help clients to make informed decisions by acting as their advocate regarding their clinical status and treatment options
    • Develop effective working relations and cooperate with multi-disciplinary team members throughout the entire case management process
    • Maintaining open communication and positive rapport with all clients
    • Record case information, complete accurately all necessary forms and produce statistical reports
    • Promote quality and cost-effective interventions and outcomes
    • Assess and address motivational and psychosocial issues
    • Adhere to professional standards as outlined by protocols, rules and regulations of the state and company

    Skills

    • Proven working experience in case management, or a related field (preferred, but not required)
    • Working knowledge of case management principles, client care and insurance / prior-authorization processes (preferred, but not required)
    • Previous experience with psychological aspects of care
    • Effective communication skills
    • Excellent organizational and time management skills
    • Familiarity with professional and technical emerging knowledge
    • Problem solving skills and ability to multi-task
    • Compassionate individual with sound teamworking capacities
    • BS degree in related healthcare field (preferred, but not required)
