The Prevention Program of Community Maternity Services is seeking a Case Planner (35 hours/week) to work with families in Albany County with children at risk of foster care placement. The Case Planner will provide case management, safety assessment, home-based parent education, service plan related transportation and crisis intervention for parents and children at risk of foster care involvement as well as families whose children are in foster care and planning for their return home.
Requirements include minimally a Bachelor's Degree. Candidate must have strong oral and written communication skills; organizational and computer skills. Experience working in the child welfare field preferred.
Applicants must have a valid NYS driver's license and car in good working condition, be physically able to go up and down stairs; carry a child of 40 lbs., be able to sit and/or stand for extended periods of time and demonstrate a full range of motion.
This is a full-time position and requires flexible scheduling. On-call responsibilities apply.
