We are currently hiring a Group Visit Nurse Manager based in Worcester. The Group Visit Nurse Manager Coordinates group visits for Kennedy Community Health patients. Fosters a team-based relationship with all members of the medical and behavioral health departments, continually working towards integration into Primary Care.

Essential Functions:- Manages the day-to-day operations of the in-person and/or hybrid group programs Monday through Friday including scheduling, staffing and leadership of group visits.
- Ensure Group visits schedules are appropriately filled, and no-show patients are re-engaged.
- Be able to assist Group Visit programs as needed in person.
- Provides a wide spectrum of services as part of a primary care team, linking patients to appropriate services that promote positive health outcomes, treatment adherence, and self-sufficiency.
- In collaboration with Clinical Program Nurse Manager, support ongoing grants and compliance with grant requirement and reporting.
- Develop organizational workflows and processes.
- Supervise any future staff.
- Ensure that appropriate clients are referred internally and within the community and provide follow-up.
- Foster collaborations between internal and external healthcare teams to ensure the delivery and implementation of an integrated model of care.
- Works in close collaboration with the Office Based Addiction Treatment (OBAT), Gender Affirming Care (GAC) and Reproductive Health teams.
- Through regular chart reviews, ensures that all services are appropriately documented in patients’ electronic health records.
- Unrestricted and current MA Nursing License
- Current Basic Life Support (BLS) certificate.
- 2 years of external experience. 1.5 years of experience at Kennedy Community Health.
