Overview:
We are seeking a compassionate and skilled Registered Nurse to join our Home Health team. As a Registered Nurse - Home Health, you will have the opportunity to provide high-quality care to patients in the comfort of their own homes. Duties:
- Conduct comprehensive assessments of patients' health status
- Administer medications and treatments as ordered by physicians
- Monitor and evaluate patients' progress
- Provide education and support to patients and their families on managing their health conditions
- Collaborate with interdisciplinary healthcare team members to ensure coordinated and effective care
- Perform various nursing proceduresmay include but not limited to tube feedings, trach care, vent care and medication administration
- Document patient information accurately and timelyon Nurse forms

• Understanding of confidentiality obligations and nursing best practices

Skills:
To excel in this role, you should possess the following skills:
- Strong knowledge of pediatric care with tube feedings, trach or vent care
- Ability to provide health coaching and education to patients

• Valid driver’s license, auto insurance and reliable transportation.

Qualifications:
- Valid Registered Nurse (RN) license in the state of South Carolina
- Minimum of 1 year of nursing experience, preferably in home health or a related field
- Excellent communication and interpersonal skills
- Ability to work independently and prioritize tasks effectively
- Strong critical thinking and problem-solving abilities

• Pediatrics: 1 year (Preferred)
• Ventilator: 1 year (Preferred)

License/Certification:

• BLS Certification (Preferred)
• RN License (Required)
