Lead with Heart - Nurturing Care Coordination

Supervise your caseload, nurturing the growth and development of LPNs and Home Health Aides, as we cultivate a harmonious team of caregivers. In your role as a facilitator of care coordination, we celebrate the unity that arises when PRN staff steps in, ensuring seamless support for your patients.

• Active RN License in the state of service
• One (1 ) Year of Clinical Experience; Hospital and/or Home Care experience preferred
• Current CPR Certification 
• Valid Driver’s License
