Health and Safety Supervisor supervises the identification and remediation of hazardous workplace conditions. Oversees the measurement and sampling of hazardous materials, and any removal of physical, biological and chemical hazards. Being a Health and Safety Supervisor investigates, analyzes, and reports on any accidents or unsafe events and suggests remedial and preventative actions. Develops a safety conscious culture by providing workforce education on first aid, emergency/safety procedures and regulations. Additionally, Health and Safety Supervisor ensures all necessary records are maintained and are in compliance with all applicable federal and state health and safety regulations. Participates in Occupational Safety and Health Administration (OSHA) inspections, providing inspectors with appropriate documents and identifying safety measures. Requires a bachelor's degree. Typically reports to a Manager. The Health and Safety Supervisor supervises a group of primarily para-professional level staffs. May also be a level above a supervisor within high volume administrative/ production environments. Makes day-to-day decisions within or for a group/small department. Has some authority for personnel actions. To be a Health and Safety Supervisor typically requires 3-5 years experience in the related area as an individual contributor. DUTIES AND RESPONSBILITIES:
Determine the types of Services required by a client based on an on-site visit where services will be delivered.
Develop an individualized service plan which will be agreed upon and signed by client or family member and the agency that includes: Type of services, supplies & equipment to be provided. Location of services. frequency and duration of services, planned date of service initiation, charges for services rendered and plan of supervision
Will maintain documentation required by the agency to demonstrate individual competence and perform yearly performance evaluation on attendants.
Perform on-site supervision of PAS attendants, with input from client or family, at least every other month and more frequently if needed to ensure the client’s continue need for the service and to ensure that the attendant is adequately delivering the authorized tasks and complying with EVV policies and procedures.
Will orient PAS attendants to their job responsibilities including, but not limited to: The philosophy and values of community integration and consumer-driven care; report of abuse or neglect; change in the client’s health condition requiring emergency procedures or health services; maintenance of documentation to demonstrate that an individual is competent in those services he or she performs.
Unlicensed persons performing “Health related tasks”, that fall within the practice of professional nursing or PAS services, shall be supervised by a registered nurse.
Train and evaluate attendants on EVV process.
Responsible for interview / processing attendant hires for their respective cases.
Will visit patient’s or potential patient(s) in their home or other appropriate setting to explain service eligibility and coordinating efforts to make potential clients and make referral to appropriate entity (MCO’s, TX DADS, DHS) This also can be performed by Intake in Home Health.
Assist with responsibilities when in the absence of PAS staff.
To prevent break in service will need to be perform attendant duties/responsibilities by assisting member/client with ADL’s and personal care tasks as needed. (Last resource)
Perform Post Hospital Visits within 72 hours after client has been released from Hospital or any other facility. (if we decide to incorporate this practice)
Enter Supervisory Visits and Annual Visits into Vesta
Plan/schedule calendar of visits for the preceding month and submit to PAS Director / Administrator for review and approval.
Create Charts for New Admissions
a. Assemble all SOC Packets.
b. Service Agreement
c. Package with all forms signed by client.
Enter information into Vesta.
a. Enter Supervisory Visits
b. Enter Annual Visits
Review Active client list for potential Home Health referrals and initiate process with intake coordinator.
Other assigned visits by immediate supervisor.
