Being a Health Educator develops, coordinates, and disseminates educational resources and materials. May provide health education outreach and consultation. Additionally, Health Educator requires a bachelor's degree in a related area. Typically reports to a manager or head of a unit/department. The Health Educator work is generally independent and collaborative in nature. Contributes to moderately complex aspects of a project.   • Job Summary:

    Health Educator identifies and prioritizes eligible consumers that demonstrate undesired health outcomes and whom have shown resistance to engagement in care and supportive services via onsite and remote. The Health Educator plays an integral role in providing education for PLWH and linking PLWH in services.

    Primary Duties & Responsibilities include, but are not limited to:

    • Conduct health education screening for all clients enrolled in the ESS program via onsite or remote.
    • Conduct individual health education / risk reduction screenings / activities via in person, teleconference or ZOOM.
    • Develop and implement health education goals as outlined in the service plans via in person, phone or teleconference.
    • Work with Case Manager to enroll clients who qualify for the program
    • Ensure communication is maintained between providers regarding the client’s medical outcomes and refer appropriately when advocacy is needed using telehealth services and case conferencing.
    • Link clients to resources internally and externally to ensure they have access to necessary services.
    • Promote clients participation in the program (group and individual health education sessions) via in person and through BH social media platforms.
    • Conduct individual educational sessions with a focus on treatment adherence and viral suppression via in person, teleconference or ZOOM.
    • Coach and mentor peers in order to help them enhance their working relationship with the clients in order to achieve progressive health outcomes.
    • Document and report on specific outlined program indicators on a monthly basis to ensure the program is on target.
    • Maintain proper documentation and document encounters on daily basis in both AIRS as well as in daily service tracking grids.
    • Other duties as assigned by supervisor.

    Qualifications/Criteria:

    · Sensitivity towards PLWHAs and chronic conditions.

    · Excellent interpersonal and communication skills. Demonstrated adequate written and verbal skills.

    · Knowledge of community resources helpful.

    · Understanding of and commitment to harm reduction theory and practice. Must demonstrate a commitment to advocacy and activism, particularly in the crises of HIV/AIDS, homelessness and drug policy.

    Education:

    · BA degree and 2 years’ experience working with PLWHA / MSM and IDU.

    · Entitlement knowledge for PLWHA

    · Knowledge of harm reduction services to high risk populations including drug users, those in recovery, the homeless and sex workers.
