Duties

The Columbia VA Health Care System is actively recruiting for a Registered Nurse, Mental Health Case Manager. As a responsible member of the Acute Hospital Team the incumbent interacts with Veterans, family members, and an interdisciplinary team of professionals. He/she utilizes clinical expertise, leadership, and creative approaches to management of mental health nursing care for general outpatient mental health patients with DSM 5 diagnoses within Dorn VA Medical Center and Community Outpatient Clinics (CBOCs). In addition, applies clinical skills of case management which emphasizes a collaborative process that assesses, advocates, plans, implements, coordinates, monitors and evaluates health care options and services so that they meet the needs of the individual patient.
