This role is a community health worker supporting the quality team and nurses in prevention and wellness visits. A trusted member of the population health team, the work may encompass basic clinical assessments based on standard criteria or protocols, data and chart abstraction, gap closure, outreach and scheduling services and referrals for individuals, diagnostic services, and preventive/annual wellness visits.
Works closely with clinicians and colleagues to identify patients who need outreach for preventive services- specific focus on high risk patients Identify and assist with scheduling preventative services for patients due or overdue Completely and accurately document in patient's electronic medical record (EMR), currently Athena and/or Cerner Identify and extract information from DHIN and outreach to assist in GAP closure for clinicians and program partners Document interactions with patients including information needed for follow up and patient navigation support Provide patients and family members with education regarding the need for follow up as appropriate for scheduled visits- i.e.- annual wellness, mammograms, etc. Actively participate in organizational quality initiatives Participate in collaborative multidisciplinary team meetings to optimize clinical integration, efficiency, and effectiveness of care delivery (e.g. Microsoft Teams virtual meetings) Demonstrate a commitment to the mission, vision and strategic goals of Beebe Healthcare and its healthcare delivery including the ability to integrate values of compassion, integrity, performance.
Skills:
Exhibit independent professional judgement within boundaries of role, but the ability to consult and engage with other team members for improved patient outcomes. Familiarity with Microsoft Office applications (i.e., Word, Excel, Outlook). Familiarity with documenting in EMR application software Ability to establish positive, supportive, and trusting relationships with and among patients, their family members, and colleagues. Ability to work collaboratively and effectively within a team. Ability to take the initiative, problem-solve, and make decisions. Having empathy and compassion for those struggling with their health and issues in their lives. Experience & Education Required:
Minimum Qualifications High School Diploma or GED. At least two years related experience with community engagement and/or health (i.e., medical assistant, classroom assistant, CHW, outreach worker, volunteer, etc.). Additional education may substitute for required experience. Preferred Qualifications: 3 years of experience in working with at risk populations. Previous experience as a CHW and knowledge of community resources highly desirable.
