Hospice Supervisor manages the resources, operations, and staff of hospice care programs. Works with families and patients to develop optimal care plans. Being a Hospice Supervisor coordinates the delivery of a patient/family assessment process and care plan development. Oversees interdisciplinary teams of medical doctors, nursing staff, and other care providers inside and outside the facility. Additionally, Hospice Supervisor maintains an up-to-date knowledge of laws and regulations of federal, state, and regulatory bodies relative to hospice care. May require a bachelor's degree in nursing, social work or equivalent. Typically reports to a manager or head of a unit/department. The Hospice Supervisor manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions. To be a Hospice Supervisor typically requires 5 years experience in the related area as an individual contributor. 1 - 3 years supervisory experience may be required.   • Responsible for the 24-hour operation and management of both Circle of Life Hospice Homes. This includes planning, organizing, directing, coordinating, and evaluating direct and indirect patient care. Assumes major responsibility for day-to-day operational affairs and leadership of both Circle of Life Hospice Homes. require delivery of skilled nursing care when needed. Will work on the unit rounding from 9-5 Saturday and Sunday, and may assist with floor staffing as needed. Able to pick up additional shifts/hours to increase hours worked if Full Time is desired.

    3 or more years of progressively experience in acute care nursing. Demonstrated leadership experience as charge nurse or in other capacity. Experience managing staff on a shift basis and recent experience in a skilled facility or as a case manager of hospice patients required.

    • Facilitates and oversees patient transfers in and out of IPU.
    • Manage scheduling and staffing.
    • Serves as a resource for all IPU needs. Assists with facilitating solutions and processes.
    • Liaison with all other disciplines and departments to ensure and exceptional delivery of care.
    • Assure compliance with regulations and contracts.
    • Organize, prioritize, and delegate to meet routine daily needs, and in crisis.
    • Utilize acuity and intensity tool to manage nurse staffing levels for patient census.
    • Maintain knowledge of current hospice nursing practices.
    • Respond to emergencies and conduct patient assessments.
    • Assists with training, counseling, and coaching of staff.
    • Support best practices with EMR and documentation.
    • Assist with writing and updating policies and procedures.
    • Practice quality communication skills: verbal, written, and electronic.
    • Coordinate conflict resolution, successful peer interactions, and coaching of clinical staff.
    • Provide patient/family and community education with effective teaching skills exhibited.
    • Participate in conferences, meetings, and in-services.
    • Collaborate with outpatient service line to ensure continuum of care services for patients.
    • Articulates needs of patients transferring between levels of care to staff and promotes continuity of care.
    • Assures continuity of care through effective scheduling of staff.
    • Serves as a liaison with other departments to resolve problems and improve efficiency and effectiveness.
    • Demonstrates compassion, empathy, and patience when interacting with patients, families, co-workers, and members of the public.
    • Represents Circle of Life to the community in a positive manner.
    • Demonstrates flexibility, versatility and a positive attitude in integrating additional duties.
    • Performs other duties as assigned.
