This is a Full-Time position with the Department of Social Services Human Services Center. For more information on the DSS, please visit http://dss/sd/gov.
This position works within an inpatient psychiatric setting and assesses and provides direct social services for adolescent patients within the licensed specialty hospital. This position and serves as the direct link between the patient, families and/or guardians and community-based private and public resources. This position consults with a multi-disciplinary team within the hospital along with collecting and analyzing patient records, and interviewing patients and stakeholders in order to write a psycho social assessment. This position utilizes all appropriate resources to arrange after care services for discharge planning.
This position works daytime hours Monday through Friday.
